What are medically tailored meals?

Medically tailored meals (MTMs) are ready-to-eat meals created around a specific diagnosis or health condition. A registered dietitian designs the menu so the sodium, carbohydrate, protein, fat, and other nutrients match a person's medical needs. For example, a heart-failure patient may receive a low-sodium menu, while a person with diabetes gets consistent carbohydrate portions.

In New York, MTMs are delivered through the Social Care Networkunder the state's 1115 Medicaid Waiver. The program recognizes that food is a health intervention, not just a social service, so eligible members can receive meals at no cost while they are enrolled.

What is Meals on Wheels?

Meals on Wheels is a community-based program that delivers meals to older adults who have trouble shopping or cooking for themselves. It is one of the most well-known senior nutrition services in the United States and is funded primarily through the Older Americans Act, with additional state and local support.

The program is focused on helping seniors age safely at home. A friendly driver delivers a hot meal, and the visit itself provides a wellness check. It is not designed to manage a specific disease, and it is generally not a Medicaid benefit.
